- Women Levels I & II















- Men Levels I & II















- Co-ed Levels III & IV















- Families Levels I & II















- Level IV Independent Living































Anyone who is struggling with substance abuse is eligible for their services.































































The following documents are needed prior to interview:































- Picture ID















- Social Security card















- Criminal Background Check















- Proof of Physical Exam (within the last 60 days)















- Prescriber Letter for Medication (on Hope Havens form)















- Proof of recent TB Test Results















- Verification of Homelessness Letter (if applicable)















- Proof of Disability (Disability aware letter, copy of check, etc.)















- Copy of Comprehensive Clinical Assessment
